This is a self-assessment tool to help clients consider their spiritual condition.
Spirituality may be used generally to refer to that which gives purpose and
meaning to L.I.F.E., or the term may be used specifically to refer the practice

of philosophy, thought process, or way of living.

• knowing & engaged in life’s meaning/purpose
• Positive about life/future
• makes sound high-minded decisions
• fully engaged with family, friends, and community
• able to forgive self and others
• engaged in core values/beliefs

SPIRITUALLY STRESSED

• knowing but neglecting life's meaning/purpose
• 2nd guessing about life/future
• arbitrarily makes poor decisions
• somewhat engaged with family, friends and community
• difficulty forgiving self or others
• straying from core values/beliefs

SPIRITUALLY DEPLETED

  • • doesn’t know/losing a sense life’s meaning/ purpose
    • holds little hope about life/future
    • makes poor decisions routinely
    • weakly engaged with family, friends, and community
    • not likely to forgive self or others
    • disregards core values/beliefs
